Supported Living People With Mental Health Needs

We offer a different supported living options for residents with mental health needs.

Hazelmead House in Swindon provides supported housing, with staff supporting residents to gain the skills they need to move on into independent living.

Jim Masters Court in Swindon offers lower-support accommodation for people with mental health needs who wish to live independently.

We also provide accommodation in other parts of the country through a partnership with the charities Mind and The Kaleidoscope Plus Group.

Hazelmead House, Swindon

Hazelmead House is an established housing project for people recovering from mental health issues. Set in Swindon‘s old town it's close to all amenities and the town centre.

The project is funded through; licence fees, Supporting People, and housing benefit.

Help and support we provide

Hazelmead House has 15 self-contained furnished rooms with shared bathrooms, a communal kitchen and lounge. Staff support residents to gain the skills they need to move on into independent living.

On arrival, residents are allocated a key worker who will help them use the Outcome Star to develop a comprehensive support plan to help them plan their future. Part of the support plan includes an agreement to meet regularly with their allocated key worker who helps with personal support and access to training and employment opportunities.

How to access the service

Applications for accommodation, for anyone between the ages of 18 and 65 are made by the Community Mental Health Teams using the current Common Referral Form. Referrals should be sent to:

Jim Masters Court, Swindon

Jim Masters Court is housing for people with mental health needs who wish to live independently. Support is provided via staff at Hazelmead House. In the Gorse Hill area of Swindon, Jim Masters Court is ideally placed for all amenities and with easy access to the town centre.

The project is funded through licence fees, Supporting People, and housing benefit.

Help and support we provide

There are twelve one-bedroom flats, a car park and landscaped areas. Support is provided by staff from Hazelmead House who visit weekly and also support residents to meetings and appointments when needed.

How to access the service

Applications for accommodation, for anyone between the ages of 18 and 65 are made by the Community Mental Health Teams using the current Common Referral Form.
