Hazelmead House is an established housing project for people recovering from mental health issues. Set in Swindon‘s old town it's close to all amenities and the town centre

Applications for accommodation, for anyone between the ages of 18 and 65 are made by the Community Mental Health Teams using the current Common Referral Form. Referrals should be sent to:

Accommodation

Hazelmead House has 15 self-contained furnished rooms with shared bathrooms, a communal kitchen and lounge.

Staff support residents to gain the skills they need to move on into independent living.

Help and support

On arrival, residents are allocated a key worker who will help them use the Outcome Star to develop a comprehensive support plan to help them plan their future.

Part of the support plan includes an agreement to meet regularly with their allocated key worker who helps with personal support and access to training and employment opportunities.

The project is funded through; licence fees, Supporting People, and housing benefit.
